Form Question Types Explained
AMP allows for the customization of Forms by enabling you to select the question type, define the response label as it will appear on the resource side, and determine how the question will be presented to the end user. Below is an overview of the different question types, along with examples of how they might be used in a form.
AMP Question Types:
Simple Text
Used for short responses.
Examples: First Name, Last Name, Vehicle Description, Exact Location.
Paragraph
Used for longer responses.
Examples: Notes, Purpose, Special Needs, What Are You Plugging In?, Return Address, Package Description.
Designed for entering an email address.
Number
Used for responses that require a numerical quantity without characters (e.g., symbols or letters).
Examples: Number of Vegetarian Meals Needed, Number of Passengers, Number of Boxes.
Money
Allows for the entry of a currency amount in USD.
Example: Merchandise Item Cost.
Yes/No
Presents a toggle switch for answering a question with "Yes" or "No."
Examples: Is the Item Fragile?, Is the Item Temperature Sensitive?, Do You Need an Operator?
Date
Allows users to select a date from a calendar.
Time
Allows users to select or manually enter a time.
Select From List
Allows the question creator to create a list of options for users to choose from. The available options can be customized and limited based on the form’s needs.
Examples: Shipping Methods (FedEx UPS, Amazon etc.), Vehicle Type (Bus, Box Truck, SUV etc), Transfer Type (Hotel to Site, Site to Hotel.)
Select From Locations
Allows the question creator to provide a full or restricted list of project locations for users to choose from. The available locations can be filtered by category or individual locations to limit what the end user sees.
Examples: Where do You need the equipment delivered?, Pickup Location.
Select From Project Dates
Allows the question creator to provide a full or restricted list of project dates for users to choose from. The available dates can be filtered by category or individual date to control what is displayed to the end user.
Examples: Date for Connect/Disconnect, Pickup/Return Date, Start/End Day.
File
Allows users to upload a file.
Example: Connector Image
Phone Number
Reformats the number input by end users to a standard national or international phone number format.
