Adding Resource-Level Questions (Reusable Across Forms)
Resource-level questions allow you to collect standardized information from requesters across multiple forms.
To add a reusable resource question:
Navigate to Resource Questions on the Resource Level [NOT the form.]
Click Add Question.

Complete the following fields:

Question Type – Select the input method (e.g., simple text, select from list, date).

Internal Label – This is how the question is tracked across forms.
Example: Use "First Name" as the internal label, but display it as "Pickup First Name" on one form and "First Name" on another.

Click Submit when finished.
Adding Questions to a Resource Form
To add questions directly to a specific resource form:
Navigate to the form.
Click the Form Questions tab.
Click Add Question.

Choose one of the following options:
Use Existing – Add a reusable question previously created at the resource level.

Create New – Add a new, form-specific question.
For this guide, we’ll walk through the “Create New” option.
Complete the following fields:

Question Type – Select the input method (e.g., simple text, email, date).

Internal Label – How the question is identified on the back-end (e.g., in the Resource Requests table).
Question Prompt – The visible question text shown to the requester on the form.
Helper Text – Optional guidance to help the requester understand what is being asked.

Is Response Required
Toggle Yes if the question must be answered before the request can be submitted.

Per Item
Toggle Yes if the requester must answer this question for each individual item they are requesting.

Is Identifier
Toggle Yes to make the response display in the requester’s list of submitted requests.
This will also add the response as a searchable column in the Approvals Dashboard, and the resource and form request Lists.
Note: Toggling this on will also automatically set Searchable to Yes.

Searchable
Toggle Yes to make the response searchable in the resource and form request lists.
Note: Static fields like portal name remain searchable by default.
