Creating an Advance Type Intake Form

An Advance Type Intake Form can be created to appear in all portals within a specific advance type. Creating an Intake Form will allow you to collect necessary information from portal groups you are managing. 

 

To Create an Advance Type Intake Form

  1. Navigate to Advance Types and Portals.
  2. Select the applicable Advance Type.
  3. Navigate to the Configuration Tab.
  4. Locate the Intake Form section.
  5. Select Add Question.
  6. Select from the available question types and add the desired questions to your form.
  7. Reorder the questions to edit how they appear on the form.

Viewing the Intake Form in the Portal

The Intake Form will appear in the portals:

  • Below messaging and instructions
  • Above the resources available for request

Permissions Needed

Users wishing to create an Advance Type Intake Form will need Editor or above permission to the advance type they are managing.
